An anonymous reporting form for speaking up
Give people a safe way to raise a concern, from fraud and safety risks to harassment, even when they can’t put their name to it. The form keeps only what they type: no name, no IP address, no device details and no cookies, so not even we can tell you who sent a report.
How it works
-
Start from the Speak up template
It explains how to stay anonymous, asks what happened and offers an optional way to get a reply.
-
Share it widely
On the intranet, in the staff handbook, or on a poster with a QR code. It works on personal phones, outside your network.
-
Send reports to the right people only
Email each report only to the people who look into them. For a tighter circle, keep the form in a separate account that only they can open.
Why it fits
Anonymous by design
We don’t store IP addresses, device fingerprints or cookies for anyone who fills in a form, so there is nothing to hand over, not even to you.
A way to reply, if they want one
People can leave an email or phone number, ideally a personal one. Without it, they stay fully anonymous.
Kept only as long as needed
Set reports to delete themselves after the period your policy allows.
Part of your reporting channel
Under the EU Whistleblower Directive, organisations with 50 or more workers need an internal reporting channel. A form like this can be how reports come in; acknowledging them within 7 days and giving feedback within 3 months stays your process.
Questions
Can you tell us who sent a report?
No. We don’t record IP addresses, device details or cookies for people who fill in forms, so we have nothing that could identify them. Only what they type is stored.
Is a form enough for the EU Whistleblower Directive?
It can be the channel reports arrive through, but the Directive also asks for a process: who handles reports, confidentiality, acknowledging within 7 days and feedback within 3 months. Check your national law; this isn’t legal advice.
How can we reply to someone anonymous?
Only if they leave a way to reach them. The template asks, and suggests a personal address they don’t use at work.
