Marketing consent forms, done properly
Newsletter sign-ups, offers by email or text message, event invitations. Ask for each kind of message separately, with boxes people tick themselves, and keep a record of what they agreed to and when. The form itself sets no cookies and tracks no one, which is a good start for a marketing list.

With a webhook through Make, n8n or Zapier, or as a CSV export. If you use double opt-in, your mailing tool sends the confirmation email.
Why it fits
One box per purpose
Emails, text messages and partner offers each get their own consent box, so people can say yes to one and no to another.
Never ticked for them
Consent boxes start empty and can’t be prefilled from a link: people always tick them themselves.
A record of what they agreed to
Each response is saved with the time it was sent, and the form’s version history keeps the exact wording of every box.
No tracking on the sign-up itself
The form sets no cookies and loads no pixels, so signing up doesn’t add anyone to an advertising audience.
Questions
Can a consent box be ticked in advance?
No. Under the GDPR, consent has to be an active choice, so consent boxes in Agent002 always start empty, and prefilled links can’t tick them.
Do you do double opt-in?
Not on the form itself. Send sign-ups to your mailing tool with a webhook (Starter and up) and let it send the confirmation email, as most mailing tools can.
How can people withdraw their consent?
As easily as they gave it: put an unsubscribe link in every message, and say in your privacy notice how to withdraw. To remove someone’s sign-up, delete their response in Responses and in your mailing tool.
